A long-term condition (LTC) is an illness that cannot be cured. It can usually be controlled with medicines or other treatments. These include:
People with these conditions need annual health check-ups to make sure their condition is stable and being managed correctly.
At your Annual Health Review (previously called CDM1) we will take a blood sample, check your blood pressure, and update other health data.
You will then need to book a review with either a nurse specialist or clinical pharmacist who will discuss your test results and go over your medications with you. they will then arrange your prescriptions for the next year. They will consider how together we could improve your health/management of your LTC. This may include health advice, medication changes or referrals to a community service.
Your review date should fall around your birthday month to make it easier to remember! A few weeks before your birthday, we will contact you by text, email, or post to arrange your appointment.

⊘ You have diabetes as one of your long-term conditions. You need to book an 'Annual Diabetic Review' appointment instead.
⊘ You have not had your annual invitation. If you are having symptoms and want to discuss these, please contact the practice to book an appointment.
⊘ You do not have any of the long-term conditions listed above. This is not an appointment with a clinician who can advise on general health issues.
