Continuity of care survey

The results of the survey will be used as part of The Health Foundation’s study on continuity of care which we have been a part of. 


Patient Participation Group

All patients are eligible to join our patient group; we would very much like our group to include members from as broad spectrum as possible to get a truly representative patient opinion. We need young and old patients, patients that work, patients that don’t work, patients that care for others, patients with long term conditions and patients from different ethnic groups.

The aims of the group are:

  • To give the practice a patients perspective on how we can improve our services
  • To improve communication between the practice and our patients
  • To exchange information and ideas
  • To provide our patients with an opportunity to be involved in how the practice is run

I want to find out more

Patient Group members are invited to attend regular meetings which give us an opportunity to really engage with our patients and understand their perspectives of our service. 

We will sometimes discuss specific topics – such as changes in the practice or particular challenges we are facing, and work together to find a solution that works for everybody. 

We may also discuss patient feedback patterns, and things that are of particular interest to the patient group.

How to get involved

To join our patient group, you must be a patient of the Practice or a carer of a patient of the Practice. 

Please send an email to mbccg.surgeryinfo.jcp@nhs.net with your full name, date of birth, and email address you prefer to be contacted on (if different to the one being used).

Our Patient Services Manager will add you to our group and will be in touch with you soon.
